​Who is Community Team?

 

The Community Team is a multi-agency group committed to developing and facilitating comprehensive, family-centered, and creative solutions to support and empower Knox County families and children facing multiple life stressors. This is accomplished through multi-agency collaboration to ensure the most effective use of existing services and the development of family specific services. Community Team updates are provided to the Council at each quarterly Council meeting.

 

Community Team is made up of 12 Knox County agencies who are also members of Knox County FCFC, and who have delegated a representative, (and a back-up person), to act on behalf of their agency at Community Team.  Community Team meets twice a month, (once in-person, and once virtual via Microsoft Teams).  During this meeting, case managers, service coordinators, and direct care staff who are working with families present information about their family to the Community Team, with the goal being to learn about community resources they may not have previously been aware of, get ideas for engaging and assisting their families, or requesting funding for their family to assist with their needs.  During meetings, coordinators are providing updates on these families and the progress being made.  The Community Team consists of members from the following agencies:

  • Knox County Board of Developmental Disabilities

  • Knox County Help Me Grow - Early Intervention

  • Mental Health & Recovery Board of Licking & Knox Counties

  • Mount Vernon City Schools

  • Behavioral Healthcare Partners of Central Ohio

  • The Freedom Center

  • New Directions 

  • The Village Network

  • Fredericktown Local Schools

  • East Knox Local Schools

​Anyone can make a presentation at Community Team and this includes the families themselves who can present their story to Community Team.  The process for presenting a family to the Community Team, completing the necessary documentation, and starting a family team can be found here.​​​​​

What is the Process for Presenting at Community Team?​​​

This is the process for requesting to present at an upcoming Community Team Meeting:

To present a family at Community Team, the FCFC Community Team Funding Request Presentation Form needs to be completed and sent to the FCFC Email Box no later than 4:00 PM on the Tuesday before the next scheduled Community Team meeting.  The FCFC Coordinator will review the request for appropriateness of the request, and to ensure it meets all FCFC eligibility criteria.  If there are questions about the request, the FCFC Coordinator will reach out to the presenter for clarification of questions.  If your request to present at Community Team is approved, the FCFC Coordinator will email you to let you know what time slot you will be scheduled to present for the next Community Team meeting. 

 

If your Community Team presentation request contains a FCFC funding request, all FCFC documentation must be current and up to date.  FCFC Plan of Care needs to be reviewed and updated at minimum, every 90 days.  CANS Assessments need to be updated every 90 days.  FCFC Release of Information forms need to be current for all members of the household.  If any of the documentation needs updated, the FCFC Coordinator will email you to advise of the necessary documentation before a presentation approval will be given.

You will be given a timeslot for your presentation at the next Community Team Meeting.  If the meeting is a In-Person Community Team Meeting, you will need to present in person.  In-Person meetings are held at Knox County Job & Family Services located at 117 E. High Street, 3rd Floor, Mount Vernon, OH 43050, in the A/B Conference Room.  If the meeting is a Virtual Community Team Meeting, you will be emailed the link for the Microsoft Teams Meeting.

Community Team Meetings can be intimidating for new presenters because of having to present in front of a panel of people you may not know.  We do our best to provide introductions of everyone in attendance and make the presenter feel as comfortable as possible.  Share as much information about the family and their needs, and try and answer all questions asked by the Community Team, as accurately as possible.  This is your time to pick the brains of the team about additional community resources and services, ask for suggestions and guidance for how to engage your family, and present requests for funding to address the families needs.  Presentations typically only last 10-20 minutes depending on the situation and information being presented.
