
How do I know if FCFC is right for my family?​
If your family is receiving services from multiple agencies, (JFS, Juvenile Court, School, Behavioral/Mental Health, Board of DD, Head Start, Help Me Grow, etc.), then they are eligible for FCFC services. Prevention is key in working with families. We want to get involved with families BEFORE they rise to a crisis level of care to hopefully prevent the need for more urgent services like residential placement. We want to be involved with the families that are receiving multiple services in order to assist in the collaboration and coordination of those services.

What's the first step to link the family to FCFC?​
The first step is a referral to FCFC. The referral form can be found HERE. We also need to have a FCFC Release of Information completed for EVERY member of the household. Once the referral form and Release of Information are completed, submit the forms to the FCFC Email Box. After we receive the referral and Release of Information forms, the FCFC Service Coordinator will reach out to the person who made the referral if we require additional information about the family, and then reach out to the family to begin the process of identifying the need and services that can assist with the need. A phone call to the family is usually initiated within 24-hours of receipt of the referral.

What Happens Next?
​Once the FCFC Service Coordinator has identified an eligible family and their need, the process of opening a Family Team begins. The FCFC Service Coordinator will meet with the family to complete the following:
• Gather family's story
• Assess for needs and strengths
• Completion of new or updated initial assessments (Knox County utilizes the CANS Assessment)
• Completion of required forms (e.g. FCFC Release of Information, FCFC Intake Screening Form, CANS Assessment, and FCFC Plan of Care)
• Identify someone already working with the family, or a new individual, to assume the role of the Service Coordinator of the Family Team.
• Assist the identified Service Coordinator with family engagement, completion of FCFC forms and updates, and technical assistance with Community Team presentations and the process for presenting at Community Team.

It is important to note that the FCFC Service Coordinator involvement is not required to open a Family Team. Any case manager, care coordinator, SSA, or individual working with a family is encouraged to make a referral to FCFC and open a Family Team when appropriate. Once the referral has been made to FCFC, the FCFC Coordinator will make contact with the person making the referral to discuss the family need. Provided the family meets the eligibility requirements for FCFC, the worker will be permitted to proceed with opening up a Family Team for their family.

​What are the Eligibility Requirements?
​The only requirements for opening a family team in Knox County are based on State guidelines for FCSS funding. FCFC Service Coordination is available to children and youth (0 through 21) with multi-systemic needs (i.e., child is not necessarily involved in two or more systems, but child's needs involve more than one system). Multiple systems is identified as receiving two or more services through county agencies, such as JFS, CPS, Juvenile Court/Probation, Behavior/Mental Health/Substance Abuse Services, Board of DD, Mental Health & Recovery Board, Head Start, Early Intervention, Primary Care Physicians, and/or local schools districts.
In addition to this requirement, a family cannot be enrolled with FCFC if they are currently enrolled with OhioRISE and receiving Tier 2/3 Care Coordination. This is considered duplication of services and we cannot provide funding for services when they are receiving OhioRISE Care Coordination. FCFC can work with a family who is receiving Aetna OhioRISE Tier 1 services but there is a process to ensure we are not duplicating services. This will be discussed on a case by case basis as this is identified with the families you are working with.

What Documentation is Needed?
The following documentation is needed to open a Family Team:
• FCFC Release of Information Form (Must be completed for every member of the household and every member 18 or older must sign the form themselves)
• Comprehensive CANS Assessment (Must be updated every 90 days)
• FCFC Plan of Care (Must be updated every 90 days)
• OhioRISE Release of Information (May be needed if OhioRISE is involved)
